Abstract

Motor vehicle accidents (MVAs) and workplace injuries can affect the body in ways that are not always easy to see during the first medical visit. A person may have normal X-rays but still experience pain, stiffness, headaches, weakness, or limited movement from injuries involving muscles, ligaments, spinal discs, facet joints, nerves, or other soft tissues. Mild concussions may also occur without clear findings on routine imaging.

After a car crash, work accident, fall, or lifting injury, emergency care usually focuses on finding dangerous problems first.

  • Severe head injuries
  • Spinal cord damage
  • Major nerve injuries
  • Life-threatening complications

This evaluation is necessary. However, a normal X-ray does not mean that every structure in the body is normal.

Plain X-rays are very helpful for looking at bones and alignment. However, they do not provide the same level of detail for muscles, tendons, ligaments, spinal discs, nerves, or joint capsules.

According to the American College of Radiology, MRI may be useful when doctors suspect ligament, nerve-root, spinal cord, or other soft-tissue injuries after spinal trauma (American College of Radiology, n.d.).

For people who continue to experience pain after an accident, the important question may not simply be, “Did the X-ray look normal?”

A better question is, “What tissues could still be contributing to these symptoms?”

Common Hidden Injuries After an MVA

Car accidents can subject the body to powerful forces in a fraction of a second.

The head may move rapidly forward and backward. The spine may twist. The shoulders may be pushed against a seat belt. The knees may strike the dashboard. Muscles may tighten suddenly to protect the body.

These forces can contribute to injuries that are difficult to see on standard X-rays.

Spinal Ligament Injuries

Ligaments are strong bands of connective tissue that help stabilize joints.

The neck and lower back contain many small ligaments that help control spinal movement.

During whiplash or another sudden impact, these ligaments may become overstretched or injured.

Possible symptoms include:

  • Neck pain
  • Lower back pain
  • Stiffness
  • Muscle spasms
  • Headaches
  • Reduced range of motion
  • Pain during bending or turning
  • Difficulty sitting for long periods

Small ligament injuries may not create a clear abnormality on ordinary X-rays.

A complete physical examination, history of the accident, neurological testing, and additional imaging may sometimes be needed when symptoms continue.

Annular Tears in Spinal Discs

The spinal discs act like cushions between the vertebrae.

Each disc contains a softer inner area surrounded by a strong outer layer called the annulus fibrosus.

Sudden compression, bending, twisting, or acceleration forces may stress these outer fibers. Small cracks or separations can form. These are known as annular fissures or annular tears.

Some annular tears cause no symptoms. Others may contribute to:

  • Neck pain
  • Lower back pain
  • Pain with sitting
  • Pain with bending
  • Muscle guarding
  • Pain that travels into an arm or leg

Annular tears may also occur with disc bulges or herniations.

Because spinal discs are soft tissues, they cannot be directly evaluated on a standard X-ray. MRI may provide additional information when medically appropriate (Tenny et al., 2023).

Facet Joint Capsule Injuries

Facet joints are small joints located along the back of the spine.

They help guide movement and prevent excessive motion.

Each facet joint is surrounded by a connective-tissue capsule. A sudden car accident or workplace injury can place stress on these capsules.

Facet-related symptoms may include:

  • Local neck pain
  • Back pain
  • Pain with rotation
  • Shoulder discomfort
  • Stiffness
  • Headaches
  • Pain when looking up or turning the head

Research into whiplash injuries has identified cervical facet joints and nearby tissues as possible sources of persistent neck pain after trauma.

These injuries may not appear clearly on standard X-rays because the painful structure may be soft tissue rather than bone.

Myofascial Trigger Points

Muscles often tighten after an accident.

This tightening is partly the body’s attempt to protect an injured area. However, prolonged muscle guarding may eventually contribute to painful trigger points.

A myofascial trigger point is a sensitive area within muscle or surrounding connective tissue.

Trigger points can contribute to:

  • Tight muscles
  • Deep aching pain
  • Shoulder discomfort
  • Headaches
  • Neck stiffness
  • Back pain
  • Reduced mobility
  • Pain that travels to another area

Castaldo et al. (2014) found that patients with whiplash-associated disorders may have active myofascial trigger points.

These painful muscle changes are another example of why a person may continue to hurt even if there is no fracture on an X-ray.

Mild Concussions

A concussion can happen even when the head does not directly strike an object.

The Centers for Disease Control and Prevention explains that a mild traumatic brain injury can occur when a blow to the head or body causes the brain to move rapidly inside the skull (Centers for Disease Control and Prevention [CDC], 2025).

Possible concussion symptoms include:

  • Headache
  • Dizziness
  • Brain fog
  • Poor concentration
  • Memory difficulty
  • Sensitivity to light
  • Sensitivity to sound
  • Sleep changes
  • Balance problems
  • Irritability
  • Nausea

Clinicians often diagnose a concussion through symptoms and clinical examination.

Routine CT or MRI imaging is not required to diagnose most mild concussions. Clinicians may use these tests when they suspect bleeding, skull injury, or another serious problem.

This means someone can have a real concussion even when structural brain imaging appears normal.

Workplace Injuries Can Develop Gradually

Not every work injury happens during one sudden accident.

Some conditions develop because the same movement is repeated hundreds or thousands of times.

Examples include:

  • Repetitive lifting
  • Frequent bending
  • Repeated twisting
  • Long periods at a computer
  • Overhead work
  • Standing for many hours
  • Reaching
  • Carrying heavy equipment
  • Using vibrating tools
  • Poor workstation ergonomics

Over time, these stresses can affect muscles, tendons, nerves, spinal joints, and other soft tissues.

This is sometimes called cumulative trauma.

A worker may gradually notice pain, numbness, weakness, stiffness, or loss of function without being able to identify one exact moment when the injury happened.

Why a Whole-Person Evaluation Matters

Pain after an accident does not always come from only one structure.

For example, a neck injury may cause the person to move differently. The shoulders may tighten. Sleep may become poor. Physical activity may decrease. Stress may increase.

Over time, several problems may begin to overlap.

A complete evaluation may examine:

  • How the accident happened
  • Where the body was positioned
  • When symptoms began
  • Range of motion
  • Muscle strength
  • Reflexes
  • Sensation
  • Balance
  • Posture
  • Walking pattern
  • Joint movement
  • Muscle tension
  • Functional limitations

Imaging can then be chosen based on the patient’s symptoms and examination findings.

No single imaging test can identify every possible accident injury.
