To get ready for your appointment please be certain that you have completed the health history questionnaire in complete detail. Please ensure that the requested documents have been signed. Completing your documents beforehand will guarantee the best possible treatment and time with our medical practitioner (s). Lastly, if you have any medical records, laboratory tests, etc., we encourage you to email or fax them to the office. If you have any further questions or concerns on how to go about these processes, we’re delighted to assist!
