I recall a time in my own life, in my early 50s, when I wasn’t feeling like myself. I was fatigued, my cognitive function felt sluggish, and my overall vitality was low. Had my physician not used a simple screening tool, that opportunity to optimize my health would have been missed. My wife, a 47-year-old former collegiate athlete with no “classic” symptoms of hormonal decline—no hot flashes, weight gain, or sexual dysfunction—felt “off.” She finally got her labs done, and her testosterone level was a staggering 12 ng/dL. A value that low is profoundly impactful on a person’s well-being, affecting everything from energy and mood to cognitive clarity and bone density.

This is why we must screen everyone. It starts with a simple, validated symptom checklist.

  • DO thank them for taking the time to fill out the form.
  • DON’T assume someone is not a candidate, either medically or financially.
  • DON’T discuss specific therapies at this stage. The goal is to identify symptoms that warrant further investigation with lab work.

The only true contraindications to BHRT are active, hormone-sensitive cancer (like breast or prostate cancer), pregnancy, or, for men, actively trying to conceive. A family history of cancer is not a contraindication; in fact, optimized hormones can be protective (Glaser & Dimitrakakis, 2013). Screen everyone.

The Consultation: Clarity, Confidence, and Connection

Once the labs are back, the consultation is your opportunity to connect the dots for the patient. This is where you bring everything together, but it is crucial to remain concise and impactful. My experience has taught me that long, drawn-out consultations often overwhelm patients rather than empower them. Aim for a 10- to 15-minute consultation.

The structure is simple and powerful:

  1. Validate Their Symptoms: “Here are the symptoms you reported on your checklist.”
  2. Present the Objective Data: “Here is what your lab work shows, confirming the hormonal imbalances.”
  3. Offer the Solution: “Based on your symptoms and labs, our dosing algorithm recommends a specific, personalized therapy to restore your balance and help you feel like yourself again.”

Patients are not looking for a complex biochemistry lecture. They are looking to you with one fundamental question: “Can you help me?” They are struggling with brain fog, joint pain, poor sleep, and low energy. They are desperate for a solution.
